Job Role: Care Coordinator

The Salvation Army is a Christian movement and one of Australia's largest charities, helping thousands find hope in personal hardship.

We are seeking an experienced Care Coordinator to join our Salvos SA Home Care team. As a Care Coordinator, you will be responsible for the ongoing coordination and management of services and supports provided to allocated Home Care Package clients.

Your key responsibilities include:

  • Undertaking initial assessment and on-boarding for new clients and maintaining ongoing assessment and exemplary relationship with clients and their representatives.
  • Developing, implementing and monitoring consumer directed care plans in collaboration with clients.
  • Providing consumer directed case management services to clients acting as the main point of contact between clients, their informal carers, Home Care Workers and external service providers.

You will also establish and maintain positive working relationships with all members of the Salvos Home Care team and with the broader Aged Care Support Services team.

To be successful in this role, you will require:

  • An appreciation of and the ability and willingness to promote, support and model the Christian values and ethos.
  • Tertiary qualifications in a relevant field or minimum of three years industry experience.
  • Current unrestricted registration with Australian Health Practitioners Regulation Agency.
  • Motivation to work with older people living in the community and an understanding of consumer directed care and enablement approaches.
  • Proven leadership experience in overseeing, coordinating and implementing consumer directed care services within a Home Care setting.
  • Excellent oral and written interpersonal skills, strong customer service focus and ability to negotiate and influence others.
  • Current drivers' licence, good driving record and willingness to travel for work.
