Job Description

As a Transition Care Program Coordinator, you will play a vital role in supporting older individuals as they transition from hospital to community-based care. Your primary responsibility will be to coordinate the acceptance of clients into the program and ensure they receive tailored support that meets their unique needs.

Key Duties
  • Liaise with hospitals to identify suitable patients for the program
  • Develop short-term care coordination plans to facilitate smooth transitions to rehabilitation services
  • Establish ongoing case management in the community and maintain linkages with aged care agencies, primary health care, and General Practice
Requirements

To be successful in this role, you will require:

  • A degree in a relevant Allied Health discipline with registration eligibility
  • Effective communication skills, including problem-solving, conflict resolution, and team collaboration
  • At least 3 years post-graduate experience in Allied Health, particularly with older adults
