Home Care Specialist

As a key member of our multidisciplinary team, you will contribute to the delivery of high-quality patient care in a dynamic and fast-paced environment.
- Contribute to safe and timely discharge and improved independence and occupational outcomes for clients
- Documenting assessment findings and interventions in a timely fashion
- Liaising with the Senior Social Work and the Director, Social Work RAP about the provision of services and resources in the occupant's work area
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers
- Undertaking training as required to attain and maintain required competency of skills and knowledge applicable to the role
About You:
- A degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
- Ability to undertake assessment and intervention with clients with a range of illness and disabilities relevant to older people and rehabilitation
- Organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately and demonstrate initiative
- Experience in the provision of clinical Social Work services to clients with a range of illness and disabilities relevant to older people and rehabilitation
This service provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission. You will participate as a member of the Virtual Rehabilitation Ward team, reporting to the Virtual Rehabilitation Ward Manager.
