Older Peoples Mental Health Care Specialist

As a key member of our multidisciplinary team, you will be responsible for providing high-quality, evidence-based clinical care to older people presenting with complex mental health issues.
This role involves working collaboratively with consumers, carers, general practitioners, aged care services, and other healthcare professionals to deliver comprehensive care that addresses the unique needs of each individual.
The ideal candidate will possess excellent communication skills, be able to work effectively in a team environment, and have a strong commitment to delivering person-centered care.
Key responsibilities include conducting thorough assessments, developing and implementing personalized care plans, and providing ongoing support and guidance to consumers and their families.
Requirements:- Current registration as a Registered Nurse, Psychologist, or Occupational Therapist with the Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for membership with the Australian Association of Social Workers (AASW).
- Collaborating with multidisciplinary teams to deliver comprehensive care to older adults with complex mental health needs.
- Conducting thorough assessments and developing personalized care plans that address the unique needs of each consumer.
- Providing ongoing support and guidance to consumers and their families, including crisis intervention and management.
- Staying up-to-date with best practices and research in mental health care for older adults.
