Key Role in Care Team Leadership

We are seeking a highly skilled and experienced leader to join our care team as a Community Lead. In this role, you will have the opportunity to empower older adults to live with dignity, independence, and connection.

About the Role

This is an exciting opportunity to lead a multidisciplinary team of professionals who are dedicated to delivering high-quality care to our clients. As a Community Lead, you will be responsible for providing clinical and operational leadership, guiding your team to deliver person-centred care that promotes wellness and reablement.

  • You will lead, mentor, and develop a high-performing team, ensuring that services meet program guidelines, quality standards, and customer requirements.
  • You will champion a restorative, wellness, and reablement approach to care, ensuring customer outcomes are central to every decision.
  • You will build and maintain strong relationships with community leaders, external partners, and customers.
Responsibilities

Your key responsibilities will include:

  • Providing clinical and operational leadership across the care team.
  • Mentoring and developing team members to achieve their full potential.
  • Ensuring high-quality care delivery that meets program guidelines and quality standards.
  • Championing a restorative, wellness, and reablement approach to care.
  • Building and maintaining strong relationships with stakeholders.
Requirements

To be successful in this role, you will need:

  • A Bachelor of Nursing degree or equivalent qualification.
  • Current registration with AHPRA (or equivalent).
  • Proven leadership experience in a community aged care or health service setting.
  • Senior case management and/or coordination experience with a strong customer service focus.
  • Excellent knowledge of aged care legislation, standards, and programs.
