- About this role
As a Care Manager, you will play a pivotal role in coordinating care plans, managing client relationships, and collaborating with our office team for seamless service delivery.
You will be based in our Unley office, working as part of a dynamic and supportive team that values collaboration and excellence in care.
Key Responsibilities:
- Develop tailored care solutions by communicating effectively with team members and clients.
- Plan, manage, and review client goals and care plans.
- Foster strong relationships with clients and their families through formal agreements regarding care services and associated costs.
- Document all interactions with clients, family, or stakeholders, and share relevant information with the team.
- A qualification in a health-related discipline such as social work, allied health, or related field.
- Prior experience working with seniors or aged care is essential.
- At least 1 year of experience in a similar role is required.
- Strong written and verbal communication skills are necessary.
- A valid Australian Driver's Licence is mandatory.
