Role Overview

The role requires managing the overall client experience and working closely with our dedicated team to ensure seamless service delivery.

Key Responsibilities

  • Plan, manage, coordinate and review client goals.
  • Communicate with team members and clients to develop care plans.
  • Formalise agreements with the client and family about the care services provided and costs involved.
  • Document all interactions with the client, family or other stakeholders about the client and share relevant information with appropriate team members.
  • Meet with clients and families to discuss and review current care plans and goals.

About You

Hold a qualification in a health-related discipline (social work, allied health or related field) and have previous experience working with seniors or aged care. A minimum of one year's experience in a similar role is required. Be highly efficient, organised, and possess strong written and verbal communication skills. A valid Australian Driver's License and Australian work rights are necessary requirements.
