Job Title: Care Partnership Specialist

Role Summary:

We are seeking an experienced Care Partnership Specialist to support our team in providing high-quality, person-centered care to clients in their own homes.

About the Role:

The successful candidate will assess clinical care needs, develop and review care plans, and deliver excellent clinical care in partnership with clients and stakeholders. They will work collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provide guidance to care staff to ensure safe and continuous delivery of high-quality clinical care.

Key Responsibilities:
  • Nursing qualification and current registration with a relevant nursing body.
  • Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
  • Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.
