Support Worker for Vulnerable Families

Role Summary:
The Case Manager - Early Intervention Family Support role involves working collaboratively with families to enhance their safety and connectedness within the community.
Key Responsibilities- Develop a family case plan in conjunction with families, incorporating initial screening and assessment results.
- Identify barriers to support carers through practical, in-home assistance.
- Coordinate case management activities, provide parenting information for carers, and implement specific supports for children to strengthen their ties to culture and community.
This position requires close collaboration with a dedicated team to achieve successful outcomes.
Key Selection Criteria Include:
- Relevant qualifications in human services or equivalent experience/skills.
- A deep understanding of Aboriginal culture and values, as well as an awareness of the current challenges faced by Aboriginal children, young people, and their families.
- Proven ability to work effectively within a team environment.
- Significant experience in case management.
- Knowledge of vulnerable children, young people, and families.
- Familiarity with attachment, trauma theories, child development, and resilience.
- Ability to collaborate and communicate effectively with various stakeholders, including community services and other agencies.
- Understanding of the child protection and out-of-home care service system.
