Job Overview

We are seeking a skilled and compassionate Case Manager to join our Early Intervention & Family Support team. As a key member of our team, you will work closely with parents and family carers to improve safety and connectedness within their community.

The successful candidate will be responsible for providing initial screening and assessment, developing family case plans, and offering in-home support to carers. You will also oversee case management activities, provide parenting information, and promote links to community and culture for children and families.

Key Responsibilities:
  • Provide case management services to families, including assessment, planning, and support.
  • Develop and implement individualized family case plans to address specific needs and goals.
  • Collaborate with other agencies and services to ensure comprehensive support for families.
  • Provide parenting education and support to carers, focusing on attachment, trauma theories, and child development.
Requirements:
  • Appropriate qualification in human services or equivalent experience and skills.
  • Knowledge and understanding of Aboriginal culture and values.
  • Demonstrated ability to work effectively as part of a team.
  • Experience in case management and working with vulnerable populations.
  • Knowledge of trauma-informed practices and attachment theories.
