Job Overview

The Transition and Post Care Support Officer is responsible for providing critical support to young people with complex needs aged 15-21 years, helping them navigate the transition to adulthood and post-care. As a dedicated professional in this role, you will play a vital part in promoting the wellbeing and independence of these individuals.

Key Responsibilities:
  • Identifying Unique Needs: You will identify and respond to the unique needs of high-risk young people, adhering to established frameworks such as the Strengthening Families Protecting Children Framework and the Aboriginal and Torres Strait Islander Child Placement Principle.
  • Goal Setting and Planning: You will promote and facilitate goal setting and planning for young people, emphasizing connection to family, culture, and community.
  • Specialist Services: You will coordinate specialist services addressing high-risk and complex needs, focusing on independent living skills, safe housing, and improved wellbeing.
  • Cultivating Relationships: You will cultivate relationships with government and non-government agencies to ensure efficient service delivery.
  • Quality Improvement: You will collaborate with regional stakeholders to enhance quality improvement and alignment with priorities.
  • Workforce Capacity: You will contribute to workforce capacity development in child safety and external agencies.
  • Record Keeping: You will maintain accurate and compliant record keeping.
