Community Homelessness Specialist

The Outreach Case Manager will work closely with individuals and families experiencing homelessness, providing holistic case management to achieve housing stability and social inclusion.
This role is responsible for delivering effective outreach case management to diverse cohorts at risk of or experiencing homelessness.
Key responsibilities include conducting comprehensive intake and assessments using a trauma-informed and Housing First approach.
- Developing individualised case plans tailored to each participant's goals, background, and needs.
- Collaborating with participants to support them in achieving their goals.
Main Responsibilities
- Delivering outreach services to individuals and families at risk of or experiencing homelessness.
- Conducting intakes and assessments to identify client needs.
- Developing and implementing case plans to address client needs.
What You Will Bring
- A tertiary qualification in Community Services, Social Work, or a related field.
- Previous experience in the homelessness sector is highly desirable.
