Job Description:

As a Part-Time NDIS Support Coordinator, you will play a pivotal role in empowering individuals with disabilities to live fulfilling lives.

Key Responsibilities:

  • Develop and implement personalized support plans that cater to the unique needs of clients.
  • Collaborate closely with clients, families, and stakeholders to assess individual objectives and provide tailored support services.
  • Facilitate the implementation of NDIS plans by connecting clients with relevant service providers and ensuring seamless coordination.
  • Maintain open lines of communication with clients, families, and stakeholders to address concerns and celebrate successes.
  • Ensure comprehensive documentation and reporting align with NDIS standards, facilitating informed decision-making and strategic planning.

Requirements:

  • Proven experience in support coordination or a similar role within the disability or NDIS sector.
  • Strong knowledge of NDIS processes and effective navigation skills to ensure smooth case management.
  • Exceptional interpersonal and communication skills to build trust and rapport with diverse clients and stakeholders.
  • Self-motivated with the ability to manage a diverse caseload independently and adapt to changing circumstances.
  • Formal qualifications in Social Work, Allied Health, or a related discipline (desired).
  • A valid driver's licence and reliable vehicle are essential for efficient field operations.
