Job Title: Empowering Lives Support Professional
We are seeking a passionate and empathetic support worker to play a vital role in empowering individuals with disabilities. As an Empowering Lives Support Professional, you will empower individuals to live independently, participate fully in their communities, and achieve their goals.
The successful candidate will be responsible for providing person-centred support services, maximising the skills and abilities of our clients, and promoting community inclusion, independence, decision making, and personal choice.
Key Responsibilities:
- Implement and train support services that promote empowerment, respect, dignity, and inclusion
- Develop and maintain relationships with clients, carers, and other stakeholders
- Work collaboratively with multidisciplinary teams to deliver high-quality support services
- Conduct needs assessments, develop individual plans, and provide ongoing support
- Monitor and report on client progress, identifying areas for improvement
- Participate in professional development opportunities to enhance knowledge and skills
Required Skills and Qualifications:
- Minimum Certificate III in Disability Studies or related field
- Committed to promoting empowerment, respect, dignity, and inclusion
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
- Highly developed written, verbal, and interpersonal communication skills
- Demonstrated ability and skill to use person-centred care
