**About the Role**

This is an exciting opportunity for a Support Coordinator to join our dynamic team. As a Support Coordinator, you will be responsible for supporting clients with NDIS plans, facilitating access to community and recreational activities, and promoting social inclusion and independence.

**Key Responsibilities**

">

- Provide support and guidance to clients in developing and implementing their NDIS plans.
- Facilitate communication between clients, service providers, and other stakeholders to ensure seamless coordination of supports.
- Develop and maintain relationships with service providers to ensure timely and effective delivery of services.
- Monitor and evaluate client progress, identifying areas for improvement and making recommendations as needed.
- Collaborate with colleagues to develop and implement strategies to improve client outcomes and enhance team performance.

**Requirements**

">

- Fluency in Auslan is an advantage, but not essential - we welcome applications from individuals who are willing to learn and develop their skills.
- Tertiary qualifications in Social Work, Psychology, Allied Health, or similar fields are highly valued.
- Certificate IV in Disability Services or similar qualification, and/or equivalent knowledge and experience in quality community services.
