The Sydney Children's Hospitals Network is a leading healthcare provider in Australia, catering to thousands of children each year. Our goal is to empower young people to live their healthiest lives.
The Trapeze service operates in close collaboration with the Department of Adolescent Medicine across The Sydney Children's Hospitals Network (SCHN). Its primary objective is to support transition for young people aged 14-25 years with chronic conditions and their families/carers, utilizing a developmental approach to independence and self-care.
Young people with chronic conditions face numerous challenges as they transition from child-centred care to adult-oriented health services. At this point, they assume responsibility for their own care, and in some cases, there is fragmentation in their treatment, reducing health outcomes.
The aim of Trapeze is to engage young people with chronic conditions in adult health services, enhancing the quality of their life and reducing unplanned avoidable hospitalisations. The service provides comprehensive support to young people as they transition, adopting an integrated coordinated care approach as the Model of Care.
Trapeze is a multidisciplinary team located at The Children's Hospital at Westmead and Sydney Children's Hospital, Randwick, comprising social workers, an adolescent physician, an occupational therapist, clinical nurse specialists, an administration officer, and a manager.
As a Transition Support Coordinator, you will provide psychosocial and care coordination interventions to improve the transition process for these young people and their families. This will include comprehensive transition assessment, individual and family support, collaboration with the multidisciplinary Trapeze team, other hospital subspecialty teams, and external stakeholders.
You will support young people with high-risk clinical conditions to optimize health outcomes by developing their capacity for self-management as developmentally appropriate.
To be successful in this role, you will require:
- A tertiary qualification in an allied health or nursing discipline
- Registration with the relevant professional body
- High-level communication skills to contribute to Youth & Transition service delivery, resource development, and clinical support
- Postgraduate experience and knowledge of adolescents living with chronic conditions, the developmental considerations involved, and associated transition to adult healthcare challenges
- Expertise in evidence-based psychosocial interventions and group work
- Experience working in partnership with a multidisciplinary team and other service providers
- Skills in care coordination and knowledge of healthcare and psychosocial resources in the community
- Competence in computer skills and electronic medical records
- A current NSW driver's licence
