About Help at Hand Support
A dedicated provider of Support Coordination services, committed to empowering individuals with disabilities to achieve their goals under the National Disability Insurance Scheme (NDIS) in Victoria.
Position Purpose:
The Support Coordinator is responsible for delivering personalized support to NDIS clients, working closely with them to implement plan objectives, build capacity, and manage the impact of their disabilities.
About You:
- Excellent verbal and written communication skills, with efficient computer skills and a focus on customer service.
- Strong interpersonal, written, and verbal communication skills, with the ability to develop good working relationships.
- Ability to apply empathy and have a passion for the sector, with good time management and organizational skills.
- Effective time management, caseload management, problem-solving skills, and ability to plan and manage resources and budgets.
- Knowledge and understanding of the NDIS, with experience in working with people with complex support and service requirements.
Qualifications & Experience:
- Minimum Certificate IV in Disability or related field, with demonstrated knowledge and experience of the NDIS.
- Strong interpersonal, written, and verbal communication skills, with the ability to develop good working relationships.
- Effective time management, caseload management, problem-solving skills, and ability to plan and manage resources and budgets.
- Knowledge and understanding of the NDIS, with experience in working with people with complex support and service requirements.
- Previous Support Coordination or Case Management experience, with a current First Aid and CPR Certificate and Australian Driver's license.
Tasks & Responsibilities:
- Establishing connections with key stakeholders, including NDIS Providers, community venues, and healthcare teams.
- Monitoring plan outcomes and expenditure to ensure services remain within the plan budget for participants.
- Independently managing a varied caseload of participants in the community, ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments and researching and referring participants to relevant services when required.
