We are seeking a dedicated and passionate Support Coordinator to join our team at Help at Hand Support Services. As a Support Coordinator, you will be responsible for providing personalized support to clients of the National Disability Insurance Scheme (NDIS).

Responsibilities:

  • Working closely with clients to implement supports outlined in their NDIS plans
  • Fostering capacity building, managing the impact of their disabilities, and enhancing their overall health, well-being, and community participation
  • Establishing connections with key stakeholders, including NDIS Providers, community venues, and healthcare teams
  • Monitoring plan outcomes and expenditure to ensure the services remain within the plan budget for participants
  • Independently managing a varied caseload of participants in the community, ensuring quality of care and meeting client objectives and requirements

Requirements:

  • Minimum Certificate IV in Disability or related field
  • Demonstrated knowledge and experience of NDIS
  • Strong interpersonal, written, and verbal communication skills
  • Ability to develop good working relationships with internal and external stakeholders, including participants, families, and health providers
  • Effective time management, caseload management, problem-solving skills, and ability to plan and manage resources/budgets
