Disability Support Specialist

We are a dedicated provider of Support Coordination services, committed to empowering individuals with disabilities to achieve their goals in Victoria.
Job Title: Disability Support SpecialistThis role involves working closely with clients to implement supports outlined in their NDIS plans, fostering capacity building and enhancing their overall health and well-being.
Responsibilities:- Establishing connections with key stakeholders, including NDIS Providers, community venues, and healthcare teams.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently managing a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments and researching relevant services when required.
- Ensuring participants are working towards achieving their goals and documenting progress in reports.
- You will have excellent verbal and written communication skills, efficient computer skills and customer service skills.
- A minimum Certificate IV in Disability or related field and demonstrated knowledge and experience of NDIS is required.
- You should be able to develop good working relationships with internal and external stakeholders and manage resources/budgets effectively.
- A competitive salary ranging from $60,000 to $80,000 per annum depending on experience.
