Housing Pathways Coordinator Position

We are seeking an experienced Senior Housing Pathways Coordinator to join our Mental Health Division at Northern Health. As a key member of our team, you will provide leadership and recovery-focused interventions that respond to the housing needs of our consumers.
Key Responsibilities- Work collaboratively with consumers, community clinicians, and families/carers to identify and support access to appropriate housing options.
- Provide consultation and support to staff within the Inpatient units to ensure comprehensive understanding of local and extended housing systems.
- Develop and maintain relationships with housing stakeholders to inform policy development and strategic work.
- Work across various sites and the division's catchment to ensure effective service delivery.
- Approved degree in Social Work or Occupational Therapy, with relevant registration and experience.
- Proven experience working within or with the housing sector, with in-depth knowledge of local and state housing service systems.
- Extensive relevant experience in mental health or acute hospital settings, with a strong understanding of the Royal Commission's recommendations.
- Excellent communication and consultation skills, with the ability to work with diverse stakeholders.
