Job Summary

We are seeking an experienced Case Manager to join our team at Launch Housing. As a Case Manager, you will be responsible for providing intensive support services to individuals and families experiencing homelessness, with a focus on housing stability and trauma-informed care.

Key Responsibilities

  • Develop and implement individualized case plans to address the unique needs and goals of each client
  • Provide ongoing support and advocacy to clients, including assistance with housing applications, benefits, and other essential services
  • Work collaboratively with internal and external stakeholders, including housing providers, healthcare services, and community organizations
  • Conduct regular home visits and assessments to monitor client progress and provide support as needed
  • Participate in regular team meetings and case conferences to ensure effective communication and coordination of services
  • Maintain accurate and up-to-date records of client interactions and progress

Requirements

  • Relevant tertiary qualification in human services, social work, or a related field
  • Minimum 2 years of experience in case management or a related field, preferably in the homelessness sector
  • Strong understanding of housing and homelessness policy, context, and systems
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with clients and stakeholders
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability
  • Valid Victorian driver's license (mandatory)
