Housing Pathways Coordinator

We are seeking an experienced Senior Housing Pathways Coordinator to join our Mental Health Division at Northern Health. As a key member of our team, you will provide leadership and recovery-focused interventions that respond to the housing needs of our consumers.
Key Responsibilities- Work collaboratively with consumers, community clinicians, and families/carers to identify and support access to appropriate housing options.
- Provide consultation and support to staff within the Inpatient units to ensure seamless care coordination.
- Develop and maintain a comprehensive understanding of the local and extended housing system, including strategic work, policy development, and housing stakeholder engagement.
- Work across various sites and the division catchment to ensure effective service delivery.
- Approved degree in Social Work or Occupational Therapy, with eligibility for membership of the Australian Association of Social Workers or current registration with the Occupational Therapy Board of Australia/AHPRA.
- Experience working within or with the housing sector, with a deep understanding of the local and state housing service system.
- Extensive relevant experience in mental health or acute hospital settings, with a strong focus on housing and community-based services.
- Knowledge of the Royal Commission into Victoria's Mental Health System Recommendations, particularly related to social housing.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
