Job Overview: Improving health access for marginalised youth and reducing reliance on Emergency Departments is the primary focus of this role.
Main Responsibilities:
- Support young people to better navigate the healthcare system by enhancing access to primary and community health services.
- Develop care pathways that reduce reliance on Emergency Departments and improve patient outcomes.
- Build trust-based relationships with vulnerable youth to identify and address systemic gaps in access to ongoing, coordinated health support.
- Collaborate with stakeholders to develop a framework that improves access for marginalised cohorts.
- Contribute to long-term strategies that enhance equity and health outcomes for at-risk youth.
Required Skills and Qualifications:
- Current APHRA Registration as a Registered Nurse/Midwife.
- Post-registration qualifications or experience working with young people from marginalised groups.
- Demonstrated high level communication, liaison, negotiation, time management, computer, and presentation skills.
About This Role: As a Clinical Nurse Specialist Grade 2, you will be part of a multidisciplinary team working collaboratively with General Practitioners, Community Services, Non-Government Organisations, and other service providers to deliver high-quality care to marginalised youth.
