Job Description

This role is a vital part of improving access to healthcare for marginalized young people, focusing on those who frequently present to Emergency Departments.

Responsibilities
  • Support marginalized young people to better navigate the healthcare system.
  • Enhance access to primary and community health services.
  • Reduce reliance on Emergency Departments by improving care pathways.
  • Build strong, trust-based relationships with young people facing barriers to care.
  • Identify and address systemic gaps in access to ongoing, coordinated health support.
  • Collaborate with stakeholders to develop a framework that improves access for this cohort.
  • Contribute to long-term strategies that improve equity and health outcomes for vulnerable youth.
Required Skills and Qualifications

Essential Criteria: Current APHRA Registration – registered Nurse/Midwife. Post-registration qualifications.

Selection Criteria: Demonstrated understanding of and experience working with young people including young people from marginalized and at risk groups. Demonstrated experience working collaboratively with other service providers including General Practitioners, Community Services, Non-Government Organisations and the ability to work within a multidisciplinary team. Demonstrated high level communication, liaison, negotiation, time management, computer and presentation skills.
