Job Opportunity:
A full-time Referral Officer position requires exceptional customer service and referral processing skills. You will need great communication skills and proven administration experience to manage ongoing referral coordination activities.
Key Responsibilities:
- Deliver high standard of customer service, including using interpreter services where required
- Confirm consent to collect and share client information, provide service fee information if required, and notify about expected first visits
- Process referrals in a timely, accurate, and thorough manner to meet contractual requirements and support best care
- Identify and gather referral information from other sources as appropriate, including updating external client portals
- Work closely with senior designated roles during routine referral processing and escalations
- Accurately document into the client database and electronic files, comply with organisational records management requirements
- Liaise with clients, their carers, health professionals, database personnel, and other referrers as required
- Seek guidance on complex referrals and take direction from supervisors
- Handle difficult callers, including those resistant to services or at risk
- Identify at-risk or vulnerable clients and escalate appropriately
- Maintain high data input accuracy and integrity
