Care Advisor Role

The Client Service Manager will provide person-centered care support to clients by building meaningful relationships and ensuring they receive the necessary care and support to lead their desired life.

Main Responsibilities:

  • Manage a caseload of consumers in the Home Care Package program, including end-to-end management of clients.
  • Respond to inbound calls from potential new customers, conduct assessments and reviews, and guide new clients through the onboarding process.
  • Ensure proactive management of client care, needs implementation, and communication in line with My Aged Care guidelines.
  • Maintain high standards of report writing/case notes and proactively manage concerns regarding clients.

About You:

  • Must have a strong understanding of My Aged Care.
  • Highly motivated with strong organizational, interpersonal, and problem-solving skills, with a strong attention to detail.
  • Excellent communication skills - both written and verbal (essential).
  • Have a compassionate approach to care and improving the quality of others' lives.

Qualifications and Experience:

  • At least two years' industry and case management experience within the health care or home care sector (essential).
  • Degree or Diploma in Community Services is not essential but would be highly regarded.
  • Current Australian Driver License (essential).
