Senior Care Coordinator

As a Disability Navigator, you will be responsible for coordinating care, managing cases and providing navigation support for people who are hospitalised to ensure they receive appropriate assistance upon discharge.
About the Role- Coordinate care, manage cases and provide navigation support for people with disabilities who are hospitalised to ensure they receive appropriate assistance upon discharge.
- Support individuals with disabilities who are at risk of hospitalisation or require expedited access to services to alleviate pressure on local healthcare services.
- 5 weeks paid annual leave per year
- Salary packaging (reduced tax liability) and enjoy up to $9k for living expenses + $2.6k for meals and entertainment
