The Disability Navigator role at our organization involves providing care coordination, case management, and navigation for individuals with disabilities in hospital to ensure they have appropriate supports for discharge.

This challenging opportunity is suited for an experienced Allied Health Clinician or Social Worker who can work as part of a multidisciplinary team to provide support to individuals with disabilities and their families.

Some key responsibilities include:

  • Providing care coordination, case management, and navigation for individuals with disabilities in hospital to ensure they have appropriate supports for discharge.
  • Working as part of a multidisciplinary team to provide support to individuals with disabilities and their families.
  • Developing and implementing individualized care plans that address the unique needs of each client.
  • Communicating effectively with clients, families, and healthcare professionals to ensure seamless transitions and optimal outcomes.
Requirements
  • Tertiary qualification in Social Work or a relevant Allied Health profession.
  • 3 years clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
  • Demonstrated advanced clinical reasoning, experience, and practice skills as relevant to assessment and treatment of people with disability.
Selection Criteria
  • Tertiary qualification and 3 years clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
  • Demonstrated advanced clinical reasoning, experience, and practice skills as relevant to assessment and treatment of people with disability.
  • Demonstrated experience working in the disability sector, with a strong understanding of navigating the NDIS and alternative disability related supports.
  • Demonstrated ability to coordinate and communicate complex care plans within a health care context, as part of a multi-disciplinary team.
  • Demonstrated ability to work autonomously and as part of a team.
