The role of a Disability Navigator is critical in ensuring seamless transitions from hospital to community-based care, expediting access to necessary supports and relieving pressure on health services.

This position involves facilitating the discharge and transition to community care for patients with disabilities, working collaboratively as an Outreach Clinician for the Specialist Intellectual Disability Health Team (SIDHT) and providing specialist care and support.

Key Responsibilities:
  • Provide comprehensive support to patients with disabilities, ensuring their safe and successful transition from hospital to community care.
  • Collaborate with multidisciplinary teams, including healthcare professionals and social workers, to provide holistic care and support to individuals with disabilities.
  • Identify and support individuals who are eligible for National Disability Insurance Scheme (NDIS) services, streamlining their access to necessary resources and supporting their independence.

This role requires advanced clinical reasoning, experience and practice skills in assessing and treating individuals with disabilities. The ideal candidate will have a strong understanding of navigating the NDIS and alternative supports, as well as the ability to coordinate and communicate complex care plans within a healthcare context.

Required Skills and Qualifications:
  • Tertiary qualification in a relevant field, such as Social Work or Allied Health.
  • At least 3 years of clinical experience in a related field, preferably in disability care or a similar setting.
  • Advanced clinical reasoning, experience and practice skills in assessing and treating individuals with disabilities.
  • Ability to work autonomously and prioritize caseloads, manage stress and effectively negotiate, advocate, collaborate and communicate with various stakeholders.
  • Capacity to complete reports using language relevant and appropriate for government agencies.
