Job Role Overview

The Disability Navigator plays a pivotal part in assessing and treating individuals with disabilities, necessitating strong clinical reasoning, experience, and practice skills.

This role involves collaborating with the Specialist Intellectual Disability Health Team (SIDHT) Outreach Clinician to provide care coordination, case management, and navigation for individuals with disabilities who are hospitalized to ensure successful discharge.

  • Care Coordination:
    • Developing individualized plans to facilitate smooth transitions from hospital to home or community-based care settings.
  • Case Management:
    • Providing comprehensive support services to address the unique needs of individuals with disabilities.
  • Navigation:
    • Aiding individuals with disabilities in navigating the healthcare system to access necessary resources and services.
Key Responsibilities

As a Disability Navigator, your key responsibilities will include:
