Job Description:

As a Disability Navigator, you will provide care coordination and case management for individuals with disabilities to ensure a successful transition from hospital to community. You will work closely with multidisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each individual.

The ideal candidate will have a tertiary qualification in a relevant field and at least 3 years of experience as an Allied Health Clinician. You will possess strong clinical reasoning skills, experience working in the disability sector, and the ability to communicate effectively with stakeholders.

Key Responsibilities:
  • Provide care coordination and case management for individuals with disabilities
  • Develop and implement comprehensive care plans that meet the unique needs of each individual
  • Work closely with multidisciplinary teams to ensure seamless transitions from hospital to community
  • Communicate effectively with stakeholders, including individuals, families, and healthcare providers
Required Skills and Qualifications:
  • Tertiary qualification in a relevant field (e.g., Social Work, Occupational Therapy)
  • At least 3 years of experience as an Allied Health Clinician
  • Strong clinical reasoning skills
  • Experience working in the disability sector
  • Ability to communicate effectively with stakeholders
