Your expertise in nursing or allied health/human services will enable you to make a significant impact in the community.
About the Role:
- Assess, plan, implement, and evaluate client care in accordance with our Tailor-Made Service Model.
- Act as a primary contact for clients throughout their care journey.
- Manage client onboarding and offboarding, ensuring seamless communication and excellent service.
Requirements:
- Extensive experience in nursing or allied health/human services.
- Sound knowledge of client-centred, community-based support.
- Strong stakeholder management capabilities.
- Ability to deliver consistently professional and innovative person-centred care.
Qualifications:
- Tertiary qualifications in health/community service or other relevant disciplines.
- Ability to satisfy probity checks by legislation or policies.
