Your part in our Blue Care team

As BlueCare Liaison, you will oversee the delivery of best practice, quality care within your neighbourhood.

Key responsibilities include but are limited to:

- Ensuring client care is assessed, planned, implemented, and evaluated in accordance with BlueCare's Tailor-Made Service Model, as well as funding models, taking into consideration each client's physical, social, emotional, spiritual and clinical needs and encouraging client/ client representative participation.
- Complete regular review of services ensuring effective schedule of client services and maximising package spends.
- Act as a key contact for clients through their Blue Care Journey - and be a day to day entry point for services and general enquiries, plan enhancements and changes to funding.
- Onboarding and offboarding clients, and management of client information in CRM, ensuring up to date details and case notes.
- Assist with orientation of new staff and ongoing professional development, and fostering and driving a high-performance culture and providing great quality of life aligned to the BlueCare values and mission.

What makes a difference for us?

You'll have extensive experience in nursing or allied health or human services and demonstrated understanding and sound knowledge of client centred, community-based support. You will have extensive stakeholder management capabilities and be able to deliver consistently professional and innovative person-centred care.
- Tertiary qualification in health/community service or other relevant discipline
- Able to satisfy the required probity checks by legislation or Blue Care policies
- Ability to liaise with clinical and care team to provide coordinated care as per the package
- Excellent written and verbal communication skills
