Employment Type: Temporary Part Time (Up to 30/06/2028) | Hours Per Week: 24 | Position Classification: Clinical Nurse Specialist Grade 2 | Remuneration: $57.11 - $58.98 per hour

Overview

Support the health of kids in care by coordinating assessments and plans in a vital liaison role across agencies in Western Sydney. The OOHC Health Pathways Program is a key initiative within Western Sydney Local Health District (WSLHD).

What You'll Be Doing

  • The OOHC Liaison Nurse facilitates and implements health assessments and health management plans for children and young people entering Out of Home Care (OOHC) within WSLHD.
  • Work within a multidisciplinary and multi-agency approach with internal and external stakeholders, including government and non-government partners.
  • Clinical, liaison, implementation and monitoring functions to support OOHC health needs.

Essential Criteria

  • Registration: Current Registered Nurse Division 1 with APHRA

Selection Criteria

  • Minimum of three (3) years post-registration experience appropriate to performing the role of a Clinical Nurse Specialist Level 2 for children and young people in OOHC
  • Currency of practice and competence in assessing children and their families in relation to health and social needs; understanding of Families NSW principles
  • High level communication, liaison, networking and negotiation skills
  • Understanding and ability to implement primary health care principles and work in partnership with families
  • Experience working collaboratively in a multidisciplinary team with other service providers
  • Ability to work independently with strong time management and problem-solving skills
