Healthcare Liaison Role Overview

The Out of Home Care Liaison Nurse is a vital part of our multidisciplinary team, responsible for coordinating assessments and plans to support the health needs of children and young people entering out-of-home care in Western Sydney.

Main Responsibilities

  • Assess and develop individualized health plans for children and young people in out-of-home care, ensuring their healthcare needs are met.
  • Collaborate with internal and external stakeholders, including government agencies, non-profit organizations, and community groups, to provide comprehensive care.
  • Provide clinical guidance, liaison services, and monitoring functions to support the health needs of children and young people in out-of-home care.

Key Requirements

  • Current registration as a Registered Nurse Division 1 with the Australian Health Practitioner Regulation Agency (APHRA).
  • Minimum three years post-registration experience as a Clinical Nurse Specialist Level 2 working with children and young people in out-of-home care.
  • Currency of practice and competence in assessing children and their families in relation to health and social needs.
