Your Role

You'll be the cornerstone of our Eastern Metropolitan Region's Case Management team, assisting individuals with complex care needs. As a vital member of our Home Care Packages team, you'll craft personalised care packages, consulting with clients, their caregivers, service providers, and other crucial figures in their lives.

Your Expertise

You'll ensure our clients receive exceptional care, right in the comfort of their homes.

Requirements

  • Tertiary qualifications in relevant health or social sciences
  • Demonstrated commitment to Annecto's Purpose and Principles
  • Previous experience in case management in a community setting
  • Able to work efficiently with culturally diverse groups
  • Excellent communication skills, both written and verbal
  • Understanding and commitment to implementing Consumer Directed Care Initiatives and person-centred approaches
  • Current unrestricted Victorian Driver's license
  • NDIS Workers Screening Check
