Coordinator for Client Empowerment

As a Support Coordinator, you will work closely with clients to help them effectively use their NDIS-funded supports, connect with appropriate service providers and allied health professionals, and track their progress toward achieving their goals.
About the RoleThis is an opportunity to make a meaningful difference in the lives of individuals and communities. As a registered, independent NDIS provider, we support over 1,500 clients across Queensland, Melbourne, and NSW.
Main Responsibilities- Support clients and families in accessing and coordinating services.
- Build relationships with internal and external stakeholders.
- Implement supports outlined in NDIS plans.
- Help clients develop skills to coordinate support and engage in the community.
- Empower clients to understand and implement their plans.
- Be responsive to client needs, especially in crises.
- Conduct regular progress reviews and report to the NDIS.
- Understanding or knowledge of the NDIS.
- Experience in support coordination or similar.
- Familiarity with psychosocial disabilities (an advantage).
- Excellent interpersonal skills and the ability to collaborate effectively.
- Strong written communication skills.
- Demonstrable problem-solving skills.
