The role of Service Coordinator is integral to the delivery of in-home support services, empowering aging clients to live independently within their own homes and communities.

About Your New Position

As a Service Coordinator, you will be the primary point of contact for our customers and lead a team of Care Workers.

Key objectives include managing a portfolio of approximately 150 customers receiving In-Home care, leading a team of Care Workers who deliver Domestic Support, Social Services, and Personal Care, and enabling your team to deliver tailored services and care that empowers clients to live happily and safely at home.

You Will Lead:

- Operational leadership of a team of Care Workers
- Assist with overall leadership support for staff and clients
- Excellence in client service delivery, including assessment of client care and goal planning, onboarding new clients, and managing escalations and complaints.
- Developing relationships with existing and new clients and their families, and collaborating with other relevant service providers.

About You:

- Previous experience managing a service area or program in a Health Care, Aged Care, or Community Service organisation
- Strong People Leadership experience developing and maintaining high-level team performance
- Managing remote direct reports and a dispersed workforce, coaching and developing individual and team performance, and managing a high-volume client caseload with complex client care needs.
