Job Description:

We are seeking a dedicated Empowerment Specialist to empower adults and children with disabilities to build skills, connect with their community, and achieve their goals.

The ideal candidate will have experience supporting individuals with disabilities, possess a strong understanding of person-centred and strengths-based practice, and be responsible for coordinating supports and life transitions.

About the Role:

  • Coordinate supports and life transitions for adults and children with disabilities
  • Support employment, community access, and independent travel
  • Build daily living and life skills with clients
  • Provide early intervention where needed
  • Develop strong relationships with local services and networks

Requirements:

  • Qualifications or experience in Disability, Social Work, Education, or related field
  • Experience supporting adults and/or children with disabilities
  • Strong understanding of person-centred and strengths-based practice
  • Excellent written, verbal, and interpersonal communication skills
  • Ability to work independently, manage time, and meet deadlines
