**Role Overview**

We are seeking a skilled professional to fill the position of Case Manager within our organization. This is an exciting opportunity for an individual who is passionate about delivering high-quality support services to individuals experiencing homelessness.

In this role, you will provide critical assistance to clients who are at risk of becoming homeless or are currently without stable accommodation. Your primary goal will be to facilitate a smooth transition towards independent and safe housing solutions.

The successful candidate will have a deep understanding of the challenges faced by homeless clients and possess excellent communication skills to establish positive relationships with clients. A strong advocate, they will ensure that client needs remain the top priority.

Key Responsibilities**

  • To work closely with clients to understand their needs and develop tailored support plans.
  • To establish partnerships with local service providers to secure suitable accommodation options for clients.
  • To maintain accurate records and communicate effectively with team members and stakeholders.

Requirements**

  • Certificate IV (or above) in a Human Services related discipline.
  • At least two years of experience working in a paid or voluntary capacity in a human services field.
  • A current Australian Drivers Licence.
