We are seeking a Family Support Practitioner to deliver an outreach model of family case-management programs to families. This is a full-time position, funded by the government until 31 December 2027, with possible extension.
Key Responsibilities- Engage families and develop case plans that address unmet needs and child protection concerns.
- Work with other professionals and agencies, utilising an ecological model.
- Facilitate social learning, community connectedness, good health, and wellbeing.
- You will have a tertiary degree in a relevant discipline, such as allied health, social work, or human services.
- You will have at least 2 years' experience providing case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework.
- You will have well-developed communication, problem-solving, and interpersonal skills.
- You will have cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds.
- You will be eligible for a Working with Children Check or professional exemption.
- You will have a current driver's licence.
- You will be legally able to work in Australia.
