We are seeking a Family Support Practitioner to deliver an outreach model of family case-management programs to families in the Toowoomba/Roma/Chinchilla region.
- This is a full-time position.
- The role is government-funded until 30 June 2027, with possible extension.
- The salary range is $85,127 - $91,588 + Super, under the SCHCADS Award.
The successful candidate will be responsible for:
- Maintaining a caseload and regularly attending and participating in case reviews.
- Engaging families and developing case plans addressing unmet needs and child protection concerns.
- Working collaboratively with other professionals and agencies to ensure families' goals are achieved.
To be successful in this role, you will need:
- A tertiary degree in a relevant discipline (allied health, social work, human services).
- At least 2 years' experience providing case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework.
- Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.
