A Clinical Nurse Consultant plays a vital role in providing timely interventions to individuals who present with co-occurring mental health and substance use issues.

About the Role
  • Collaborate with multidisciplinary teams to provide comprehensive assessments and develop individualized care plans.
  • Work with clients to identify and address their unique needs, including psychosocial assessments, drug and alcohol assessments, and review of current treatment and investigations.
  • Develop and implement assertive community follow-up programs to support clients after discharge from emergency departments.

The ideal candidate will have a strong background in nursing, preferably in addiction medicine or a related field. They will possess excellent communication and interpersonal skills, as well as the ability to work effectively in fast-paced environments.

Key Responsibilities:

  • Assess and prioritize client needs in collaboration with multidisciplinary teams.
  • Develop and implement individualized care plans that address complex client needs.
  • Provide education and support to clients and their families on mental health and addiction issues.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care.
  • Maintain accurate and up-to-date records of client interactions and care plans.
