About the Role
This care coordinator position is an exciting opportunity to join our team and make a real difference in the lives of patients with complex needs. The successful applicant will be embedded into our Mental Health AOD Hub team, working closely with Team Leaders and other healthcare professionals to provide comprehensive assessment and care coordination services.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals. They will also have a strong understanding of mental health and addiction principles, as well as experience in acute settings.
We are looking for someone who is passionate about delivering high-quality patient-centered care, with a focus on timely and effective management of ED episodes and discharge planning. If you have a background in nursing or allied health, and are motivated by the prospect of making a positive impact on people's lives, we would love to hear from you.
Key Responsibilities:
- To deliver timely and comprehensive assessment and care coordination services to patients with complex needs
- To work collaboratively with patients, carers, and other healthcare professionals to achieve optimal outcomes
- To develop and implement effective care plans that address the individual needs of patients
- To facilitate smooth transitions between hospital and community settings
Requirements:
- Nursing qualification or allied health qualification relevant to the cohort
- Minimum 2 years' experience in a relevant acute setting
- Current AHPRA registration (where applicable)
- Eligibility for membership of professional association
- Current Victorian Driver's License
