Senior Care Coordinator

We are seeking a highly skilled and compassionate Senior Care Coordinator to join our team. As a key member of our care delivery team, you will play a pivotal role in ensuring the highest level of care for our residents.

About the Role:

You will be responsible for providing exceptional social and emotional support to all residents, coordinating their care needs, including medication management, nutrition, and hydration. You will lead and supervise care on the shift within designated wings or areas, performing medicine-related tasks according to your scope of qualification, state legislation, and relevant policies.

  • Support coordination of care needs, including medication management and nutrition

Requirements:

To be successful in this role, you will need:

  • A relevant qualification such as Diploma of Enrolled Nursing, Certificate IV in Ageing Support, or Certificate III in Individual Support
  • Commitment, integrity, honesty, and trustworthiness
  • Effective communication and interpersonal skills
  • Ability to understand and use information technology and programs
