This role plays a critical part in our efforts to address homelessness, by supporting clients to develop and implement personalized plans that promote their well-being and independence.

Key Responsibilities:

  • Establish strong relationships with clients to encourage self-determination and participation
  • Conduct regular assessments of client needs and circumstances, from initial intake to exit
  • Identify and proactively manage client risks, working closely with operations coordinators as needed
  • Develop and implement individualized case plans, incorporating interventions tailored to each client's needs and goals
  • Support clients in accessing relevant services and building community networks
  • Maintain accurate and up-to-date client records, adhering to TSA standards

Requirements:

  • Minimum Diploma level in Community Services or Mental Health, with relevant industry experience
  • Current First Aid certification
  • Valid driver's license
  • Willingness to undergo a national police check
