Community Support Worker

We are seeking a compassionate and experienced individual to provide psychosocial support to young people aged 17-22 in Fremantle. This is a casual position with shifts available across a 24/7 roster, including days, evenings, weekends, and overnight.

About the Role
  • Provide support to consumers to develop their own support plans in partnership with community mental health services and carers.
  • Liaise and collaborate with consumers, their community, and community support agencies such as mental health, housing, and other services to ensure an integrated and timely service.
  • Monitor and review consumers' progress through the service and against the case plan and provide up-to-date progress reports to partner organisations.
Key Responsibilities
  1. Support consumers to achieve their goals and improve their well-being.
  2. Collaborate with consumers, their families, and carers to understand their needs and preferences.
  3. Develop and maintain accurate case notes and files.
  4. Work effectively with colleagues and stakeholders to deliver high-quality services.
Required Skills and Qualifications
  • Certificate IV in Mental Health.
  • Previous experience working with people who have a mental health issue and who are homeless or at risk of homelessness.
  • Demonstrated ability to work with people from diverse cultural backgrounds, particularly Aboriginal and Torres Strait Islander peoples.
  • Demonstrated ability to positively engage and interact with people who have complex needs.
  • Current Senior First Aid Certificate and current Driver Licence.
