
Young Peoples Homeless Service Support Specialist

We are seeking a compassionate and experienced individual to provide psychosocial support to young people aged 17-22 in Fremantle. This is a casual position with shifts available across a 24/7 roster, including days, evenings, weekends, and overnight.
About the Role- Provide support to consumers to develop their own support plans in partnership with community mental health services and carers.
- Liaise and collaborate with consumers, their community, and community support agencies such as mental health, housing, and other services to ensure an integrated and timely service.
- Monitor and review consumers' progress through the service and against the case plan and provide up-to-date progress reports to partner organisations.
- Support consumers to achieve their goals and improve their well-being.
- Collaborate with consumers, their families, and carers to understand their needs and preferences.
- Develop and maintain accurate case notes and files.
- Work effectively with colleagues and stakeholders to deliver high-quality services.
- Certificate IV in Mental Health.
- Previous experience working with people who have a mental health issue and who are homeless or at risk of homelessness.
- Demonstrated ability to work with people from diverse cultural backgrounds, particularly Aboriginal and Torres Strait Islander peoples.
- Demonstrated ability to positively engage and interact with people who have complex needs.
- Current Senior First Aid Certificate and current Driver Licence.
