Job Description

Our Family Support and Accommodation Service provides individuals and families who are homeless or at risk of becoming homeless with transitional housing and supporting services.

We help clients secure long-term stable accommodation while addressing the issues associated with their homelessness and current circumstances.

  • Respond to referrals of individuals and families from internal and external services and conduct initial assessments of suitability for support
  • Undertake formal face-to-face assessments of clients, including completing all necessary paperwork using the approved assessment tool
  • Work with clients to create individual support plans, including referrals to supplementary services as needed
  • Maintain professional boundaries of care practices while building the client's capacity
  • Provide intensive ongoing case management sessions with clients and review progress against individual support plans
  • Link clients to relevant support services to address issues associated with their homelessness
  • Undertake a range of case management duties to support client development, including referrals and support letters, interaction with other service providers, appointment setting and advocacy
Required Skills and Qualifications

To be successful in this role, you will need:

  • Certificate IV or Diploma in a relevant human/community service field with at least three years' experience working in the sector
  • Experience and/or knowledge of issues surrounding homelessness, including alcohol and other drugs, family and domestic violence, unemployment and mental health
  • Demonstrated case management experience, including assessment, case work, accurate and timely recording of case notes, advocacy and referral
  • Comprehensive knowledge and ability to network and build relationships with housing, accommodation and community service providers and relevant government agencies
  • Ability to work with a range of client cohorts, including individuals, youth, couples, families, women and children
  • Knowledge of family systems, with a focus on clients who are homeless and experiencing family and domestic violence, substance misuse and mental health issues
  • Excellent communication and interpersonal skills, including verbal and written communication, report writing and statistical collection
  • Excellent relationship-building skills and demonstrated ability to work as part of a team
  • Well-developed time management skills and ability to manage a range of tasks at any given time
  • Strong computer literacy in Microsoft Office suite, as well as other systems required to support the work
  • Full driver license with no restrictions
