Job Summary:

We are seeking an experienced professional to join our housing support team in Western Australia.

About the Role:

The ideal candidate will be responsible for conducting initial assessments of suitability for support, developing holistic case plans that address clients' cultural strengths and identified needs and goals, and providing ongoing case management sessions to review progress against individual support plans.

Key Responsibilities:

  • Provide ongoing case management sessions to review progress against individual support plans;
  • Link clients to relevant support services that assist in addressing homelessness issues.

Requirements for Success:

To succeed in this role, you will need:

  • A Certificate IV or Diploma in a human/community services field with at least three years' experience working in the sector;
  • Experience and/or knowledge of homelessness issues, including alcohol and other drugs, family and domestic violence, unemployment, and mental health;
  • Demonstrated case management experience, including assessment, accurate and timely recording of case notes, advocacy, and referral;
  • Excellent communication and relationship-building skills;
  • Well-developed time management skills and the ability to manage multiple tasks simultaneously;
  • Strong computer literacy in Microsoft Office suite and other required systems;
  • A full driver's license with no restrictions.
