Complex Case Manager

We are seeking a highly skilled and experienced Complex Case Manager to join our multidisciplinary team. As a key member of our team, you will play a critical role in providing specialised case management services to veterans and their families.

Your primary responsibility will be to manage a caseload of 25-35 clients, providing high-quality care and support tailored to their individual needs. You will work collaboratively with colleagues and stakeholders to develop and implement effective care plans, ensuring the best possible outcomes for our clients.

To be successful in this role, you will need significant experience working with complex mental health clients, as well as strong case management skills. A qualification and/or registration with AHPRA as a Psychologist, RN (Mental Health), Occupational Therapist or Social Worker (AASW) is also required. While an understanding of the challenges faced by those who have served in the Australian Defence Force is beneficial, it is not essential. What is essential is your ability to provide expert support to clients experiencing the impacts of substance abuse, mood and anxiety disorders including PTSD and trauma.

In addition to your clinical expertise, you will need excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including peers, senior clinicians and clients. You will also require a current Police Check or the ability to obtain one.

Requirements:
  • Significant experience working with complex mental health clients
  • Strong case management skills
  • Qualification and/or registration with AHPRA as a Psychologist, RN (Mental Health), Occupational Therapist or Social Worker (AASW)
